It is a 0–100 momentum signal: 50% subscriber-growth velocity relative to mods in the same game, 20% favorite growth, 20% view growth, and 10% update recency. It is not a quality rating. The current score is 56.2.
Are these the real Steam Workshop numbers?

How is the rank computed?
ModOrNot compares tracked active XCOM® 2 mods by their current stored subscriber totals. Rank #1 has the most subscribers; equal totals share a rank. The rank is calculated when this page loads and can change as counts or the catalogue change.
