It is a 0–100 momentum signal: 50% subscriber-growth velocity relative to mods in the same game, 20% favorite growth, 20% view growth, and 10% update recency. It is not a quality rating. The current score is 87.1.
Are these the real Steam Workshop numbers?

How is the rank computed?
ModOrNot compares tracked active Barotrauma mods by their current stored subscriber totals. Rank #1 has the most subscribers; equal totals share a rank. The rank is calculated when this page loads and can change as counts or the catalogue change.
